Fractional Medical Director
Empowered Living Solutions (ELS) Great Falls, Montana | Contract / Part-Time | A Few Hours Per Month | Remote-Friendly Within Montana
The Opportunity
This is not a traditional Medical Director role, and that’s the point. ELS is a growing behavioral health organization building a complete continuum of community-based housing and care for adults with serious mental illness. Today, we operate Adult Foster Care homes serving individuals on the SDMI waiver. Our next step, Mental Health Center licensure, unlocks mental health group homes, day treatment, and expanded community services. One requirement stands between us and that license: a qualified medical director. This is a fractional, contract-based role by design. Montana regulation (ARM 37.106.1907) expressly permits the medical director to serve under contract, and the duties are advisory and oversight-focused rather than direct-care. Day-to-day clinical service delivery is supported by established local health system resources in Great Falls. This role is built for leverage, not volume: maximum system-level impact per hour of your time. Your impact will be felt in the moments that matter most, when a person leaves an institution after years of waiting because a community bed finally exists. As licensed services come online, both the scope and compensation of the role are expected to grow. because the license itself unlocks the revenue that funds a larger clinical program.
The role grows as the organization does.
What You’ll Do (per ARM 37.106.1907(2))
- Coordinate with and advise ELS staff on clinical matters
- Provide direction, consultation, and training on programs and operations as needed
- Serve as liaison with community physicians, hospital staff, and other professionals and agencies regarding psychiatric services
- Ensure quality of treatment through participation in our quality assurance process
- Review and approve the policy and procedure manual annually, jointly with the administrator (ARM 37.106.1908)
- Receive and help address medication error and adverse drug reaction reports
Who Qualifies (per ARM 37.106.1902)
A physician licensed by the Montana Board of Medical Examiners with any one of the following: A three-year residency in psychiatry; or Three or more years of post-graduate psychiatric training in an AMA-approved program; or Three or more years of experience in a medical practice dedicated substantially to serving persons with serious mental illnesses

Compensation & Growth
Competitive monthly stipend/retainer commensurate with the fractional scope, negotiable based on experience and availability. As ELS’s licensed services come online, the role can expand in hours, scope, and compensation: including a path toward a broader clinical leadership position for a physician who wants one.
